Lafayette Policy – Frequently Asked Questions



HOW DO I TAKE A LOAN FROM MY POLICY?

If your policy permits a loan and you have funds available, you can request the loan several ways:

  • If your tech savvy: Go online and complete the Individual Policy Service Request.
  • If your old school (or need more than $25K):   Complete the Service & Loan form and email, fax or snail mail it to FiscalBridge for processing.

If it’s less than $5,000: CalLafayette and request the loan over the phone.  Fast and efficient way!

If you prefer to have the Loan Funds direct deposited into your bank account, please complete the Authorization For Electronic Funds Transfer form, include a VOID check and submit it. Going forward, simply indicate (or advise) “Direct Deposit” on your loan request and the transaction will be processed as requested.


HOW DO I KNOW MY LOAN AVAILABILITY?

Loan details, as well as loan availability, are displayed online in your Lafayette account. Or you can call Lafayette or contact FiscalBridge.

HOW CAN I PAY BACK MY LOAN?

Call Lafayette to get your loan payoff amount.

  • Paying in full: Write out your check and mail your payment. Reference the policy number and write the words “Loan Repayment” on the check. Make sure this is done promptly after you received your payoff amount as it is only good for 10 days.

  • Paying monthly: Complete the Pre-Authorized Withdrawal form. Include the monthly amount you want them to draft and submit it via fax, email or sending to FiscalBridge.

You can also send in loan payments as you are able, just make sure to always include your Policy Number and the words “loan repayment” on your check.

You will receive a loan interest statement prior to your policy anniversary date and you can pay the annual interest off that statement.


HOW CAN I TRACK MY LOAN INTEREST?

Call or email Lafayette and ask them for a ‘Loan History’ Report.

HOW CAN I ADD MONEY (CASH VALUE) TO MY PUA RIDER (PUAR)?

If your policy is able to hold additional funds (meaning you haven’t maxed out the cash value rider), you have two options:

  • Paying monthly: Complete the Pre-Authorized Withdrawal form. Include the monthly amount you want them to draft and submit it via fax, email or sending to FiscalBridge.
  • Paper Check: You can also send in money to cash value as you are able, just make sure to always include your

Policy Number and the words “LPUA Premium” on your check.

OW DO I CHANGE MY ADDRESS?

  • Most efficient: Call Lafayette and provide them with your new address.  FiscalBridge will be notified of this change and update our records accordingly.
  • If your tech savvy: Go online and log into your Lafayette Account.

OW DO I CHANGE MY BENEFICIARY?

OW DO I CREATE MY ONLINE ACCOUNT?  

Go to www.llic.com

HOW DO I SET UP MY ONLINE ACCOUNT WHEN I AM THE OWNER OF THE POLICY BUT NOT I’M NOT THE INSURED?

When creating the account, use the Owner’s social security number (or TIN if it’s a business) and the Insured’s date of birth.

TIP: With any correspondence you are sending Lafayette, always include your policy number!

TIP: Always specify payment instructions when sending funds to Lafayette or speaking with them over the phone. If you want to add money to your cash value, make sure they know the money needs to be “added to your cash value.”
